An unannounced monitoring inspection was completed by the Licensing Specialist on 7/25/19 from 11:20 a.m. to 2:14 p.m. The current census is six (6) youth. The following is a listing of the activities for this inspection: One youth record was reviewed. This facility?s staff list was reviewed, no new staff have been hired since the February 2019 inspection. The Program Manager and Regional Supervisor were available and accessible during the inspection. An exit meeting was conducted with the Program Manager, Regional Supervisor, and the Program Development Specialist (accessible by phone) on 7/25/19 at approximately 2:04 p.m. The Acknowledgement of Inspection form was signed and left at the facility. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services website within 5 business days.
