Comments:

An unannounced renewal inspection was conducted on July 25, 2019 from 9:55 a.m. to 1:15 p.m. The agency reports a total of seven youth in placement. During this inspection, two youth records were reviewed: one current and one discharged youth record. Two new (2) staff were hired since the last inspection. Two personnel records were reviewed, to include background investigation information. Preliminary inspection findings were reviewed at the conclusion of the inspection with the Director of Independent Living Arrangements. An acknowledgement form was signed.
