Technical Assistance:
Technical Assistance was provided regarding standard 22 VAC40-131-190.V, specifically related to documentation of safety measures for medications of other household members.

Comments:
An unannounced monitoring inspection was conducted on March 4, 2020 from 9:15 a.m. to 2:30 p. m at HumanKind located at 908 North Thompson Street Richmond, Virginia 23230. HumanKind continues to provide foster care, treatment foster care and short term foster care services. At the time of the inspection the agency reported five (5) children in care and six (6) approved provider homes.

During the inspection, the following activities transpired:

1. Reviewed two (2) provider home records (entire record) (includes one new foster home)
2. Reviewed one (1) foster child record
3. Reviewed one (1) staff record (includes one newly hired staff)
4. Background checks and references for all board members were reviewed at the last inspection
5. A physical plant inspection was completed at the last inspection
6. Interviews with agency representatives were conducted

An exit meeting was conducted with the Richmond Office Program Manager on March 4, 2020 at 2:00 p.m. to discuss preliminary inspection findings. There was one (1) citation for violations of the Standards for Child Placing Agencies. Upon receipt of the violation notice, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which non-compliance will be corrected.

The licensee has five (5) calendar days from receipt of the inspection documentation to complete the section entitled "Plan of Correction", sign each page of the Plan of Correction and return it to the Licensing Office. The licensee retains a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) days, regardless of whether or not the Plan of Correction has been completed.

Violations:
Standard #: 22VAC40-131-180-F
Description: Based on the review of the record for Foster Home (FH) 1 and based on an interview with an interview with an Agency Representative (AR), the licensee failed to conduct an interview with all individuals who reside in the home of the applicant.

Findings:
1.Based on the application located in the record for FH1 and based on the home study located in the record for FH1, Other Household Member (OHM) 1 resides in the home.
2.At the time of the inspection, there was no documentation that an interview was conducted with OHM1.
3.During the exit interview, AR1 reviewed the documentation located in the record for FH1, acknowledged that OHM1 resides in the home and acknowledged that there was no documentation of an interview with OHM1.

Program Manager will add roommate interview. Program manager will date home study and assure all adult household member interviews are documented on future home studies. Effective 03/05/2020
