Technical Assistance:
22VAC40-131-370.C.3: The licensee was encouraged to reframe from writing on documents during their auditing process to avoid them appearing to be altered after signatures have been affixed.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
An announced mandated monitoring inspection was initiated on October 28, 2020 and concluded on November 5, 2020. The Program Director was contacted by telephone to initiate the inspection. The agency reported 10 children in care, 9 approved foster provider homes and 1 newly hired employee. The inspector emailed the Program Director a list of items required to complete the inspection. The inspector reviewed 2 complete child records, 3 complete foster home provider records and 1 employee records submitted by the agency to ensure documentation was complete.
Information gathered during the inspection-determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the agency. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of correction," sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public website within five business days, regardless of whether or not the Plan of Correction is completed.

Violations:
Standard #: 22VAC40-131-370-M-1
Description: Based on the review of the records presented for Foster Home (FH) 1 and based on an exit interview with agency representatives (AR), the licensee failed to document in the file of the home provider observations and findings from monitoring visits to the home.

Findings:
1.The home study presented for review for FH-1 documented that there were firearms kept in the home.
2.The information presented for review contained documentation of "TFC, Unannounced/ Scheduled Visit Documentation" forms dated 04/01/2020, 06/30/2020 and 09/14/2020. Each form documented that there were no firearms located in the home.
3.The home study dated 03/31/2020 documented that the provider home did contain firearms.
4.During the exit interview, AR2 reported that she physically observed the firearms in the provider home. AR2 reported that the "TFC, Unannounced/Scheduled Visit Documentation" forms dated 04/01/2020, 06/30/2020 and 09/14/2020 were documented incorrectly.

Plan of Correction: 22VAC40-131-(5)-370-M-1
An addendum to the ?Unannounced/Scheduled Visit Documentation? form dated 09/14/2020 was filed.
Program Manager will re-train staff on Standard 22VAC40-370-M-11. They will understand the importance of the Standard in documenting and maintaining the foster home in good standing.
