Comments:
An unannounced monitoring inspection was conducted on 12/5/19 from 9:50 am to 5:35 pm at the Farmville location of United Methodist Family Services (UMFS). Personnel records were reviewed on 12/10/19 from 10:20 am to 2:15 pm at UMFS in Richmond Virginia (location of personnel records).

The agency reports 13 approved provider homes and 18 children in care. During this inspection, three provider home records and two foster child records reviewed. Two additional provider home records were reviewed for compliance with background checks. Two personnel records were reviewed.

The community based services supervisor and regional director were available for the inspection and participated in the exit interview to discuss preliminary findings for the provider homes and children records which commenced at 4:55 pm on 12/5/19. The vice president or programs participated in the exit interview by telephone.

The human resources recruitment and talent mmanager and human resources coordinator were present for the exit interview to discuss the findings for the personnel records which took place on 12/10/19 from 2:00 pm to 2:15 pm.

There was one violation of the Standards for Licensed Child Placing Agencies. See the violation notice on the Department?s public web site for violation of the Standards.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:
Based on a review of the record for Foster Home (FH) FH1 and interview with the community based services supervisor, the licensee failed to obtain a signed authorization.

Findings:
FHI was approved by the licensee on 9/5/19. The application for FH1 documents a previous application was submitted to another child placing agency. FH1's record did not document a signed authorization allowing the previous agency to release information.

During an interview with the community services supervisor, it was acknowledged that a signed authorization was not obtained from FH1.

Plan of Correction: The South and West Region of UMFS will ensure all requests for release of information are completed when the application is submitted. All previous agencies will be contacted the prospective foster parent applied to or was
approved, denied, and or closed by, prior to considering approval of the home. The family/recruitment team will review all parent applications during monthly meetings to ensure accuracy and compliance with licensing standards.
