Comments:
An unannounced monitoring inspection was conducted on September 12, 2019, from approximately 9:40 am to 4:15 pm at the Fredericksburg office of United Methodist Children's Services (UMFS). Personnel records were reviewed at the Richmond office(location of personnel files) of UMFS on September 17, 2019 from approximately 9:35 am to 11:05 am. The regional director, assistant program director and program manager were available for the inspection and present for the exit interview in the Fredericksburg office which commenced on 9/12/19 at 3:30 pm. The human resources recruitment and talent manager and the human resources coordinator were available for the inspection and present for the exit interview at the Richmond location which commenced at approximately 11:00 am on 9/17/2019. During this inspection, 3 foster home records, 2 foster child records and 5 personnel records were reviewed. Background checks for 2 additional provider homes were reviewed. There was one citation for violation of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Violations:
Standard #: 22VAC40-131-330-J
Description: Violation: Based on a review of the record for Foster Child (FC) FC1 and interviews with the program manager and assistant program director, the licensee failed to ensure each written description of contacts made with FC1 were signed by the staff person who made the contacts. Findings: FC1 was placed with the licensee on 07/01/2019. At the time of review the contacts made with FC1 for the month of July and August 2019 were not signed by the staff person who made the contacts. During the exit interview the program manager and assistant program director acknowledged the contact notes were out of compliance.

Plan of Correction: UMFS just began using a new Electronic Health Record. Contact notes were behind for this client. All contact notes will be entered into the file by 9/19/19. From this point forward, the Program Manager will implement a more frequent check of contact note completion in the Electronic System, including checks twice a month. The Assistant Director will also provide monthly oversight in the form of reviewing the electronic files for this social worker?s cases.
