Comments:

An unannounced monitoring inspection was completed on 11/25/2019 from 10:00 a.m. to 12:30 p.m.. The licensee reports a total of six foster youth placed in independent living arrangements. Personnel records for two new staff hires were reviewed. The record for two foster youth were reviewed. An exit interview was conducted at the conclusion of the inspection with the Assistant Director (by phone) and Program Manager. An Acknowledgment of inspection was signed and provided to the agency at the end of the inspection. There were no citations for violations of the Standards for Licensed Child Placing Agencies.
