Type of inspection: Renewal An unannounced, mandated renewal inspection was conducted on 01/30/2024 from 11:15 a.m. to 3:15 p.m. The Acknowledgement of Inspection form was emailed to Director of Independent living for signature. Number of youth in ILA: 9 The licensing inspector completed a tour of the physical space that included the office setting and conditions; additionally, a tour of an IL youth?s apartment was conducted.. Youth records reviewed: 1 Number of staff records reviewed: 3 Number of interviews conducted with staff: 3; Background checks for board officers and 50% of personnel records have been reviewed during the license period. A preliminary exit interview was conducted with the Director of Independent Living, Regional Supervisor and Program Manager on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Additional Comments/Discussion: The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary.
