Type of Inspection: Monitoring An unannounced, mandated inspection was conducted on the following dates: 05/29/24. The inspection was concluded on 05/29/24. A preliminary exit interview was conducted with the Director of Independent Living and the Assistant Director of Independent Living on 05/30/24 by Teams video call. During the meeting findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Youth Census: 9 Number of youth records reviewed: 1 Number of staff records reviewed: 3 Number of staff interviews: 2 Additional Comments/Discussion: The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
