Comments:

A monitoring inspection was initiated on 10/12/2021 concluded on 10/15/2021. The Treatment Foster Care Director was contacted by telephone to initiate the inspection and reported that the current census was 11. The inspector emailed the Treatment Foster Care Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed three (3) foster child, four (4) staff, and three (3) provider home records. In addition, caseworker caseloads, on-call availability, and licensed capacity were inspected. An exit interview was conducted with the Treatment Foster Care (TFC) Director, two TFC Supervisors, Family Services Specialist Sr. Manager, Sr. Manager of Foster Care Operations, Sr. Client Operations Manager, Sr. Documentation Specialist, TFC Sr. Family Consultant, TFC Foster Family Training Supervisor, Admissions Manager, and Regional Supervisor on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description:	Violation Based on record reviews for Foster Child 1 and 2 (FC1 and FC2) the agency failed to address an element in the written assessment. Findings: 1) The written assessments for FC1 and FC2 did not include a target date for planned achievement of the child?s permanency goal. 2) The findings were discussed during the exit interview and staff acknowledged the findings.
