Comments:

A monitoring inspection was conducted on 1/6/2022 and 1/7/2022. The Treatment Foster Care Director reported that the current census was 11. The inspector reviewed two (2) foster child, three (3) personnel, and two (2) provider home records. An exit interview was conducted on 1/7/2022 the Client Operations Manager, Family Services Specialist Sr. Manager, Sr. Manager of Foster Care Operations, Treatment Foster Care Supervisor, Treatment Foster Care Foster Family Training Supervisor, Admissions Manager, Treatment Foster Care Supervisor, and Regional Supervisor where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description:	Violation: Based on record review, the agency failed to obtain a tuberculosis screening report containing required elements for Household member 1 (HH1) in Foster Home (FH2). Findings: 1. The tuberculosis screening for HH1 in FH2 was conducted and the forms were signed by medical professionals. 2. The medical professional did not mark the statement to indicate whether HH1 was free from tuberculosis in communicable form. 3. The findings were discussed during the exit interview and staff reviewed the document. 4. Staff acknowledged the findings.

Description:	Violation Based on record review and interviews, the most recent criminal history record check for Personnel 1 (P1) was not completed before three years since the dates of the last criminal history record check. Findings: 1. The personnel record for P2 contained criminal history search results dated 12/01/2016 and 1/24/2020. 2. The Licensing Specialist emailed the Staff 10 to confirm the dates of the criminal results provided to the Licensing Specialist. Staff10 confirmed the dates. 3. The findings were communicated to staff participating in the exit interview.
