Comments:
An unannounced monitoring inspection was conducted on August 6, 2019 from 9:45 a.m. to 4:25 p.m. The agency reported 26 children in placement, 21 approved foster homes, and one new staff hire since the last inspection. During the inspection, three child, three foster home, and one personnel record were reviewed. An entrance interview was completed with the Child-Placing Supervisor (S1). An exit conference was completed from 3:30 p.m. to 4:25 p.m. The Director (S1), Child-Placing Supervisor (S2), Regional Supervisor (S3), Documentation Manager (S4), and two Caseworkers (S5 and S6) participated in the exit conference. An Acknowledgment of Inspection form was completed and left with the agency prior to departure. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed. There was one citation for a violation of the Standards for Licensed Child Placing Agencies.

Description: Violation: Based on a review of the record, the agency failed to gather, review, and document a list of the strengths of the child's birth family prior to accepting the child for placement. Findings: Foster Child 1 (FC1) was accepted for placement 4/5/2019. Documentation in the child's case record did not indicate the agency received or reviewed information about the strengths of the child's birth family prior to accepting the child for placement. The finding was discussed with staff participating in the exit conference. Agency staff acknowledged the birth family strengths were not documented prior to accepting the child for placement

Plan of Correction: In response to the 22VAC40-131-(6)-250-G-4 violation, TFC Director will provide training to all TFC employees on gathering, reviewing, and documenting a list of the strengths of the child?s birth family prior to accepting the children for placement.
