Comments:
A monitoring inspection was initiated on 7/27/2021and concluded on 7/29/2021. The Sr. Manager of Client Operations was contacted by telephone to initiate the inspection and reported that the current census was 16. The inspector emailed the Treatment Foster Care Director and Regional Supervisor a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed two (2) foster child, three (3) staff, and two (2) provider home records. Caseloads and the on call schedule submitted by the agency were also reviewed. An exit interview was conducted with the Treatment Foster Care (TFC) Director, Sr. Manager of Client Operations, TFC Regional Supervisor, TFC State Admissions Manager, TFC Family Services Supervisor, TFC Documentation Supervisor, and Foster Family Trainer Supervisor on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.

The information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description: Violation:

Based on record review for Foster Child 2 (FC2), the agency failed to report to the licensing representative an injury that required medical attention beyond first aid within 24 hours following the injury.

Findings:
1) The case record for FC2 contained a Serious Incident Report documenting an injury to the child on 4/7/2021.
2) The report documented the child was taken to an urgent care medical provider. The medical provider observed and documented an injury and treatment.
3) The Licensing Specialist did not have documentation that this report was received.
4) The findings were discussed during the exit interview.
5) Staff did not locate documentation indicating the report had been submitted to the Licensing Specialist and acknowledged the findings.
