Comments:

An onsite renewal inspection was conducted on 3/4/2022 from 10:00 a.m. to 2:15 p.m. An entrance conference and tour of the office were conducted with Treatment Foster Care Supervisor. The agency reported a current census of 14 children in placement and 13 approved homes. The inspector reviewed two (2) foster child, two (2) personnel, and two (2) provider home records. During the license period 50% of personnel records were reviewed. Background checks for Board Officers were reviewed 11/4/2021 during a renewal inspection of another office location. An exit interview was conducted with two Regional Supervisors, Treatment Foster Care Supervisor, Documentation Supervisor, Sr. Client Operations Manager, and the Sr. Manger of Foster Care Administration on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined no non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the facility.
