Technical Assistance:
During the exit interview discussion occurred regarding which agency staff should be included on the agency's staff list when submitting applications for renewals or opening new offices. Discussion occurred around the definition of "involved in the day-to-day operations" found in 22VAC40-191-10. General questions regarding the possible addition of parental placement adoption services were also discussed.

Comments:
An unannounced monitoring inspection was completed on 10/24/2019 from 10:35 a.m. to 3:20 p.m. The licensee reported 18 children in foster care and nine approved provider homes. There was one new hire since the last inspection. During this inspection two foster child, one foster home and one personnel records were reviewed. The Child Placing Supervisor for this office location was available for the entrance conference, remained available during the inspection to answer questions, and participated in the exit interview. The Director, Documentation Manager, one Regional Supervisor, two Family Consultants, and two Sr. Family Consultants participated in the exit interview. The exit interview was conducted from 2:15 p.m. to 3:20 p.m. During the exit interview, the Director noted the agency is considering expanding the services offered to include parental placement adoption services. An Acknowledgement of Inspection form was signed and left with the licensee at the end of the inspection.

There was one citation for a violation of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:

Based on record review, the agency failed to ensure that there were at least seven days between face-to-face contacts with foster child 1 (FC1) each month.

Findings:

During June 2019 the caseworker completed two face-to-face contacts with FC1. The contacts occurred on 6/14/2019 and 6/18/2019. The finding was discussed with all staff participating in the exit interview. Agency staff acknowledged the finding. The non-compliance was isolated to the month of June 2019.

Plan of Correction: In response to the 22VAC40-131-(6)-330-G-1 violation, TFC Director will provide training to all TFC employees regarding face-to-face contacts needing to have seven days in between visits in the calendar month. TFC Director will have supervisors review visits during biweekly supervision with staff.
