Comments:
This is a non-mandated monitoring inspection conducted in response to an agency self-report. Due to the outbreak of COVID-19, the inspection was conducted in the form of a desk review on 6/11/2020, 6/12/2020, and 6/15/2020. Standards reviewed involved children?s services and provider homes. One (1) provider home record and two (2) child records were reviewed. Intercept Health Wytheville reported nine (9) provider homes and 17 children in placement. The findings were discussed during the exit interview and staff reviewed the social history documentation and acknowledged the findings. One violation was cited as a result of the inspection.


An exit interview was conducted by phone with the Treatment Foster Care Director, two Regional Supervisors, a Family Services Supervisor, a Treatment Foster Care Supervisor, a Documentation Supervisor, and the Senior Manager of Foster Care Administration on 6/15/2020 at 3:00 p.m. An acknowledgement form was not signed as the inspection was conducted off-site.

Description: Violation:

Based on record reviews for Foster Child 1 and 2 (FC1 and FC2), the agency failed to include all required elements in each child?s social history.

Findings:

The case record for FC1 contained a report from a Children?s Residential Facility (CRF) received by the agency on 2/26/2020. The report documented information regarding the child?s developmental and educational history, the child?s history as a victim of abuse and neglect, and prior placement at the CRF. Answers for these elements in the child?s Social History were documented as ?unknown.?

The case record for FC2 contained a Comprehensive Evaluation from a mental health services provider dated 42 days prior to the child?s placement with the agency. The evaluation contained information regarding emotional and psychological problems the child experienced within the previous 13 months and professional treatment received. The record contained a release of information form dated the day of the child?s placement allowing access to the information. The agency failed to request the information prior to completing the child?s Social History. The information was not included in the Social History.

Plan of Correction: In response to the 22VAC40-131-(6)-260-B violation, TFC Director will provide training to all TFC staff regarding the need to incorporate all required elements in each child's social history.
