Comments:
An unannounced monitoring inspection was conducted on 9/10/2019 from 10:40 a.m.to 3:15 p.m. The licensee reported 13 children in foster care and 13 approved provider homes. There were no new hires since the last inspection. During this inspection two children?s records and three foster home records were reviewed.

A Treatment Foster Care Supervisor was available for the entrance conference, inspection, and participated in the exit interview. The Regional Foster Care Supervisor, Family Services Supervisor, and a Foster Home Caseworker were available during the inspection and participated in the exit interview. The Director and Documentation Supervisor participated in the exit interview by Skype. The exit interview was completed on 9/10/19 from 2:45 p.m. to 3:15 p.m. An Acknowledgement of Inspection form was signed and left with the licensee at the end of the inspection.

There was one citation for a violation of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:

Based on record review and interview, the agency failed to conduct at least one face-to-face contact each calendar month for foster child 1 and 2 (FC1 and FC2).

Findings:

Case records for FC1 and FC2 documented the children as being approved and receiving treatment foster care services. No face-to-face contact in the foster home setting was documented for July 2019 for these children. The finding was discussed with agency representatives during the exit interview. Agency representatives reviewed the children's case files and no information was located to indicate that contacts occurred in the foster home during July 2019. Agency representative acknowledged the required contacts were not documented in the case record.

Plan of Correction: In response to the 22VAC40-131-330-G.3 violation, TFC Director will provide training to all
TFC employees regarding making at least one face-to-face contact in the foster home during the calendar month. TFC
Director will have supervisors review monthly contacts during biweekly supervision.
