Comments:
An unannounced monitoring inspection was conducted on 2/18/2020 from 9:30 a.m. to 3:00 p.m. The licensee reported 23 children in foster care and 13 approved homes. During this inspection three (3) child, three (3) foster home, and three (3) personnel records were reviewed.

A Senior Family Consultant participated in the entrance conference, remained available during inspection, and participated in the exit interview. The Foster Care Supervisor was available during the inspection and participated in the exit interview. Two Regional Directors participated in the exit interview by videoconference. The Documentation Manager was interviewed during the exit interview.

The exit interview was conducted from 2:40 p.m. to 3:00 p.m. An acknowledgement form was completed and left with the agency at the end of the inspection.

There was one citation for a violation of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:

Based on record review and interview, the agency failed to address all required components in the Home Study for (FH3).

Findings:

The Home Study for FH3 documented the applicant as currently separated from her spouse. The separation was verified by a hand written statement the applicant provided to the agency. The Study did not document verification of the couple?s marriage. The findings were discussed with agency staff during the exit interview. The Documentation Manager was interviewed during the exit interview regarding this finding. Agency staff acknowledged the findings.

Plan of Correction: In response to the 22VAC40-131-(5)-180-J violations, TFC Director will provide training to TFC supervisors and staff regarding addressing required components in the Home Study to include documenting verification of a couple's marriage.
