Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An unannounced monitoring inspection was conducted on 10/6/2020 and 10/7/2020. The agency reported 19 children in foster care and 11 approved provider homes. During this inspection two (2) child and three (3) provider home records were reviewed.

The TFC Director participated in the entrance conference and remained available during the inspection. The TFC Director, Regional Director, TFC Client Operations Manager, Documentation Manager, Documentation Specialist, FFT Supervisor, TFC FSS Supervisor, TFC Supervisor, TFC Senior Family Consultant, and TFC Family Services Specialist participated in the exit interview on 10/7/2020.

There was one (1) citation for a violation of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:

Based on record review and interviews, the agency failed to document written justifications that addressed the impact Foster Child 1 (FC1) may have on Foster Child 2 (FC2) and Foster Child 3 (FC3).

Findings:

The record for FH3 documented FC1 was placed in the home at the time FC2 and FC3 were placed. All children were receiving treatment foster care services. Written justifications for the placement of more than two (2) children were in the record for FH3. The written justifications indicated no safety concerns for any of the children because the children displayed appropriate behaviors towards one another.

The case record for FC1 documented behaviors that may impact other children. The written justifications did not address how documented behaviors of FC1 were considered and assessed when determining they would not impact FC2 and FC3.

During the exit interview, additional interviews occurred with staff S2, S3, S8, S9, and S10. Staff provided additional information confirming the behaviors were discussed during a staffing meeting prior to the placement decision for FC2 and FC3. Staff stated behaviors of FC1 were discussed with the local departments of social services holding custody of FC1, FC2, and FC3. All parties agreed placement of the children together was acceptable. Staff reviewed the written justifications and other documentation in the record for FH3 and acknowledged the findings.

Plan of Correction: In response to the 22VAC40-131-(5)-240-B violation, TFC Director will provide training
to all TFC staff regarding the requirement of documenting written justifications that
address the impact of children coming into the TFC program that will be placed with other TFC clients.
