An unannounced monitoring inspection was conducted on 3/30/2022 from 11:30 a.m. to 3:50 p.m. An entrance conference was conducted with the Treatment Foster Care Supervisor and a census of 29 children and 16 approved provider homes was reported. No new staff were hired since the last inspection. The inspector reviewed three (3) foster child and two (2) provider home records. An exit interview was conducted with the Treatment Foster Care Supervisor, Family Services Specialist Sr. Manager, Sr. Manager of Foster Care Operations, Client Operations Manager, two Regional Supervisors, Family Services Supervisor, and Documentation Supervisor on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined non-compliance(s) with applicable standards or law, and one violation was documented on the violation notice issued to the facility. Upon receipt of the Violation Notice, the licensee is encouraged develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed

Description:	Violation Based on record review, the agency failed to address required elements in the written progress review for Foster Child 2 (FC2). Findings: 1) A written progress review for FC2, for the quarter ending 3/1/2022, did not address the child?s progress towards reaching each goal and objective, a summary of contacts between the child and family, participation of birth parents, medical needs during the period under review and medical treatment received, or the child?s assessment of their own progress. 2) The findings were discussed during the exit interview. 3) Staff reviewed the written progress review. 4) Staff 1 and 7 (S1 and S7) acknowledged the findings.
