Comments:

An unannounced monitoring inspection was conducted from 11:50 a.m. to 4:02 p.m. on October 31, 2019. At the time of the inspection, the agency reported seven youth placed in independent living arrangements. During this inspection, one personnel record and two youth records were reviewed. An exit meeting was conducted with Staff S1 to discussed preliminary inspection findings before leaving the agency on October 31, 2019. An acknowledgment form was signed.
