Comments:

An unannounced renewal inspection was initiated with a site inspection on 7/12/2019 from 10:00 a.m. to 11:45 a.m. Personnel records were review remotely on 7/16/2019 and 7/22/2019. The licensee reports 6 youth in Independent Living. There were new hires since the last inspection. During this inspection 1 youth record and 13 personnel records. Background checks for all Board Officers/Agents were reviewed. The Program Director was available for the site inspection. An Acknowledgement of Inspection form was signed and left with the licensee at the end of the site inspection on 7/12/2019. An exit interview as conducted with the Executive Director by phone on 7/23/2019 from 12:00 p.m. to 12:20 p.m. There were no citations for violations of the Standards for Child-Placing Agencies.
