Comments:

A monitoring inspection was initiated on 10/21/21 and concluded on 10/29/2021. The Executive Director was contacted by telephone to initiate the inspection. The Executive Director reported that the current census was four (4) youth. The Licensing Specialist emailed the Executive Director a list of items required to complete the remote documentation review portion of the inspection. The following is a listing of the activities for this inspection: Reviewed one current resident record. Five personnel records, which consist of new staff, were reviewed. The Executive Director was interviewed on 10/27/21. The Manager of Compliance and Development was interviewed on 10/27/21. Upon receipt of this violation notice, a plan of correction is requested for the violation. The Plan of Correction should include, as appropriate: - steps to correct noncompliance of a regulation, - measures to prevent reoccurrence of noncompliance, - person(s) responsible for implementing each step and/or monitoring any preventive measure(s), and - the date by which the noncompliance will be corrected. Be advised that the Violation Notice, including the Plan of Correction and this Summary page will be posted on the VDSS web page and available for review by the general public. Do not write any names or other confidential information into your plan of correction. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services? website within 5 business days.

Standard #:	22VAC40-191-50-A-1-a
Description:	Violation: Based on interviews and review of staff?s, S3?s, background checks, the facility failed to require documentation of satisfactory background checks. The sworn statement or affirmation for S3 was not fully completed. Findings: 1) There are four questions on the sworn statement or affirmation. Each question requires a response. 2) S3 did not respond to any of the questions listed on the sworn statement or affirmation. 3) S7 was interviewed in regard to S3?s sworn statement or affirmation. S7 acknowledged the form is incomplete.
Plan of Correction:	Executive Director will ensure the HR department is fully trained and aware of the required documentation as it pertains to background checks to include full completion on the sworn disclosure statement.
