Comments:

A monitoring inspection was initiated on 11/12/21 and concluded on 11/16/21. The program manager was contacted by telephone to initiate the inspection. The program manager reported that the current census was 21 children in care and 11 approved provider homes. The inspector emailed the program manager a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed 3 children records, 2 provider home records, 4 staff records, and background checks 1 applicant. The program manager was interviewed during the inspection. An exit interview was conducted with the program manager on 11/16/21 at approximately 4:30 pm where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description:	Violation: Based on review of background checks for other household member (OHM) OHMI, the licensee failed to ensure a search of the central registry was completed within 30 days of the person in the home becoming 14 years of age. Findings: The face sheet for Provider Home (PH) PH2 documents Other Household Member (OHM) OHM1 became 14 years of age on 10/8/2021. The central registry request for OHM1 was notarized on 10/29/21 and requested on 11/4/21. At the time of the inspection the results of OHM1 central registry search was not completed.

Description:	Violation: Based on review of the background checks for the applicant, the licensee failed to ensure the search of the central registry was completed before the end of 30 days after the change. Findings. The background tracking sheet provided for Applicant (A) A1 documents A1 was appointed as a corporate board officer on 8/30/2021. The central registry search for A1 is dated 11/8/21 which is beyond 30 days of A1 being appointed.
