Comments:
An unannounced renewal inspection was conducted on August 6, 2019 from 10:00 AM - 12:40 PM. Family Preservation Services reports a total of 1 provider home and 1 foster child in placement. During this inspection, one Board Officer record, five personnel records, one provider home record, and one foster child record were reviewed. One personnel was interviewed during the inspection. An exit interview was conducted at the conclusion of the inspection with the Program Supervisor to review preliminary inspection findings. An acknowledgement form was signed. There were three violations. Upon the receipt of the violation notice, the licensee should identify the necessary corrective action and develop a plan of correction for each violation. The plan of correction should include the following: The steps to correct noncompliance with the standard(s); measures to prevent re-occurrence of noncompliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five (5) calendar days from receipt of the inspection documentation to complete the section titled Plan of Correction, sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days, regardless of whether the Plan of Correction is completed.

Standard #: 22VAC40-131-190-O
Description: Violation: Based on a review of the record for a provider home (PH-1) and an interview with personnel (S-1) the agency did not address required elements in the provider's (PH-1) evacuation plan. Evidence: The evacuation plan for PH-1 did not address: 190.O.1 How to shelter in place, when the emergency situation requires for sheltering in place, 190.O.2 How to evacuate, if evacuation is necessary. Personnel (S-1) acknowledged that this information was not captured in the evacuation plan for PH-1.

Plan of Correction: Program Director will update the emergency evacuation plan to include 190.0.1 How to shelter in place, when the emergency situation requires for sheltering in place, 190.O.2 How to evacuate, if evacuation is necessary.

Standard #: 22VAC40-131-280-C-1
Description: Violation: Based on a review of the record for a provider home (PH-1) and an interview with personnel (S-1), the agency failed to use the code of ethics and mutual responsibilities agreement in language and format approved by the State Board in the placement agreement for a foster child (FC-1). Evidence: The code of ethics and mutual responsibilities form signed by PH-1 upon placement of FC - 1 was outdated. Personnel (S-1) acknowledged that the currently approved language and format was not used.

Plan of Correction: Existing Code of Ethics form will be updated to include items C6 and B5 which were missing elements upon audit. New Code of Ethics form will be signed and replaced in FC-1 file and 2017 version will be used for all upcoming placements.

Description: Violation: Based on a review of the 14 day plan for a foster child (FC-1) and interview with personnel (S-1), the agency failed to develop and implement individualized, measurable objectives and strategies within 14 days describing services to be provided to the child's placement family during the first 45 days of placement. Evidence: The Initial Treatment Plan in the record for FC-1 did not contain individualized, measurable objectives and strategies describing services to be provided to the child's placement family during the first 45 days of placement. Personnel ( S-1) agreed that services to be provided to the placement family were not addressed in the plan.

Plan of Correction: Initial Treatment Plan will be updated to include individualized, measurable objectives and strategies describing services to be provided to the child's placement family during the first 45 days of placement. The ISP for FC-1 will be updated and placed in the file. The new ISP will be used for all future placements.
