Comments:
An unannounced monitoring inspection was completed on October 31, 2019 from 9:10 a.m. to 1:00 p.m. The licensee reports two children in foster care and four approved provider homes. There was one new hire since the last inspection. During this inspection one child, one foster home and one personnel records were reviewed. The Foster Care Supervisor, a Family Consultant, the Documentation Manager, and a Regional Supervisor participated in the entrance conference, were available during the inspection for questions, and participated in the exit interview. An additional Family Consultant and Treatment Foster Care Consultant Trainee participated in the entrance conference. The Director participated in the exit interview by phone. The exit interview was conducted from 12:20 p.m. to 1:00 p.m. An Acknowledgement of Inspection form was signed and left with the licensee at the end of the inspection.

There was one citation for a violation of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Based on record review, the agency failed to include all required elements in a quarterly progress review for Foster Child 1 (FC1).


Findings:

The quarterly progress review dated 9/28/2019 for FC 1 did not address the child?s assessment of his own progress. The finding was discussed with agency staff during the exit interview. Agency staff reviewed the quarterly report and acknowledged this information was missing.

Plan of Correction: In response to the 22VAC40-131-(6) violation, TFC Director will provide training to all TFC employees regarding appropriate documentation in the quarterly report to address the child's assessment of their own progress as well as including all other required elements needed in the report.
