Technical Assistance:
Discussed the tour of the independent living arrangement setting as noted in 22VAC40-131-440.D.8.

Discussed the beginning and ending dates of the review period for a 90 day review as noted in 22VAC40-131-440.L.5.

Comments:
A monitoring inspection was initiated on 9/2/21 and concluded on 9/13/21. The Executive Director was contacted by telephone on 8/30/21 to initiate the inspection. The Executive Director reported that the current census was six (6) youth. The Licensing Specialist emailed the Executive Director a list of items required to complete the remote documentation review portion of the inspection.

The following is a listing of the activities for this inspection:
Reviewed one current resident record. Five personnel records were reviewed, which consist of the new staff members hired since the last monitoring inspection. The Manager of Compliance and Development was interviewed on 9/9/21. The Assistant Director was interviewed on 9/9/21. The Human Resources Compliance and Training Coordinator was interviewed on 9/10/21.

Upon receipt of this violation notice, a plan of correction is requested for the violation. The Plan of Correction should include, as appropriate: - steps to correct noncompliance of a regulation, - measures to prevent reoccurrence of noncompliance, - person(s) responsible for implementing each step and/or monitoring any preventive measure(s), and - the date by which the noncompliance will be corrected. Be advised that the Violation Notice, including the Plan of Correction and this Summary page will be posted on the VDSS web page and available for review by the general public. Do not write any names or other confidential information into your plan of correction. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services? website within 5 business days.

Violations:
Standard #: 22VAC40-131-160-B-2
Description: Violation: Based on interviews and review of staff?s, S1?s, personnel record, the facility failed to include a copy of educational degree and credentials in the personnel record.

Findings:
1) S1 has occupied the Overnight Youth Support Specialist position for 39 days. The education and experience section of the agency?s job description for this particular position states the following ? ?The Overnight Youth Support Specialist shall have a baccalaureate degree or a high school diploma with at least one to two years experience working with children and/or families.?
2) Upon review of S1?s educational background that was submitted for review, only a copy of a high school diploma was provided. The resume and job description do not include experience working one to two years with children and/or families. Both documents reflect S1 was pursuing a final course for a bachelor?s degree.
3) Staff, S7, was interviewed on 9/9/21 in regard to S1?s experience working with children and/or families in relation to those with a high school diploma as noted on the job description. S7 stated S1 has a bachelor?s degree. S7 advised when one has a bachelor?s degree that experience working with children and/or families is not necessary.
4) The Licensing Specialist requested additional information to confirm that S1 has a bachelor?s degree. A college transcript for S1 was provided in response to this request.
5) Staff, S5, was interviewed on 9/10/21 in regard to the college transcript that was provided because it does not state that S1 completed the requirements for the bachelor?s degree. S5 advised that S1 finished her bachelor?s degree, but does not have a copy of the degree.

Plan of Correction: Executive Director will ensure Human Resources is fully trained on the expectations of what is included in a personnel file to include copy of educational degree and credentials.
