Comments:
This inspection was completed by two Division of Licensing Program Consultants. A monitoring inspection was initiated on 5/15/2024 and concluded on 5/21/2024. The Director was contacted by email to initiate the inspection. The Director reported that the current census was eight. A list of items required to complete the inspection was provided to the Director. One resident record and two staff records were reviewed, as submitted by the facility to ensure documentation was complete. An exit interview was conducted with Director on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Inspection findings will be posted to the Department?s public website within 5 days of receipt of the inspection documents. The department's inspection findings are subject to public disclosure.

The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.

Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.

Based on a record review and interview, the agency failed to document required information in the Social History for Foster Child 1 (FC1).

Evidence:
1)The Social History for FC1 documented:
?FC1 maintained contact with her father
?The phone number for FC1?s father
?FC1 was in contact with her aunt.
?Data fields on FC1?s Social History form for education, occupation, medical, and
psychiatric history of the child?s parent stated ?[FC1] did not report information. There is
no further information noted.?
2)The record did not document attempts to obtain the information from available sources.
3)Findings were discussed with Staff 3 (S3) and S3 was interviewed during the exit interview.
4)S3 acknowledged the findings.

Description: Violation:
Based on record review and interview, the agency failed to obtain a medical examination report addressing all elements for Foster Child 1 (FC1).

Evidence:
1)The record for FC1 contained a physical examination report, dated 11/16/2023, that did not
address the child?s growth and development, allergies, chronic conditions, or disabilities.

2)FC1 was interviewed on 2/8/2024 and provided information about allergies, chronic
conditions, disabilities, and development.

3)C1 was placed with the agency on 2/9/24.

4)The findings were discussed during the exit interview and Staff 3 (S3) was interviewed.

5)S3 acknowledged the findings.

Standard #: 22VAC40-131-340-E-1
Description: Violation:
Based on record review and interview, the agency failed to address all required information in the Service Plan for Coster Child 1 (FC1).

Evidence:
1) The Service Plan for FC1 did not include a comprehensive assessment of the child?s dental needs.
2) The quarterly report for FC1 for the period of 29/24 to 5/9/24 documented assessment of FC1s dental status and dental care within 21 days of the child?s placement.
3) The findings were discussed during the exit interview and Staff 3 (S3) was interviewed.
4) S3 acknowledged the findings.
