Comments:

An announced initial inspection was completed by the Licensing Specialist on 9/24/19 from 9:30 a.m. to 3:58 p.m. On this day, fourteen personnel files for this particular location, which are maintained at the corporate office in Richmond, were reviewed. Also, an announced initial inspection was completed onsite on 10/1/19 from 10:50 a.m. to 12:21 p.m. On this day, the Licensing Specialist toured the office space and a furnished apartment to ensure they were compliant with the applicable standards. On 9/24/19, the Executive Director and the Human Resources Compliance and Training Analyst were available and accessible during the inspection. An exit meeting was conducted with the Executive Director and the Human Resources Compliance and Training Analyst at approximately 3:40 p.m. The Acknowledgement of Inspection form was signed and left at the corporate office. On 10/1/19, the Regional Supervisor and the Senior Life Skills Coach were available and accessible during the inspection. An exit meeting was conducted with the Regional Supervisor and the Senior Life Skills Coach at approximately 12:15 p.m. The Acknowledgement of Inspection form was signed and left at the facility. This applicant is currently licensed to provide Independent Living Arrangements at other locations; therefore, policies and procedures have been previously reviewed. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services website within 5 business days.

Description:	Violation: Based on interviews with staff and review of a personnel record, the facility failed to comply with maintaining, staff, S13?s background checks before three years since the dates of the last sworn statement or affirmation, most recent central registry finding and most recent criminal history record check report. Findings: 1) The Licensing Specialist reviewed S13?s personnel file and discovered the following- a. The sworn statement or affirmation, most recent central registry finding and most recent criminal history record check report have all expired. 2) The Licensing Specialist interviewed S12 about these background checks. a. S12 was not able to provide background checks conducted before three years since the dates of the last sworn statement or affirmation, central registry finding, and criminal history record check.
Plan of Correction:	Executive Director will ensure that the HR department is fully trained on ILA expectations and regulations of background checks to meet the deadline of three years of the last sworn statement or affirmation, most recent central registry finding and most recent criminal history record check report.
