Technical Assistance:
Licensing inspector discussed caseload requirements for full time caseworkers per 22VAC40-131-80-Licensed Capacity and Maximum Caseload Numbers with the program manager.

Comments:
An unannounced monitoring inspection was conducted on May 5, 2022 from 8:55 am to 11:30 am at the office of Presbyterian Homes and Family Services, operating as HumanKind, located in Danville, Virginia. The agency reports 12 approved provider homes and 18 children placed for foster care services.

During this inspection, 2 provider home records, and 2 children records were reviewed. Background checks for one additional provider home was reviewed. One staff was interviewed during this inspection.

The program manager was available throughout the inspection. The program director and program manager participated in the exit interview to discuss preliminary findings, which commenced at 11:05 am on 5/5/2022. An acknowledgement form was left with the program manager on 5/5/2022.

There was one (1) citation for violation of the Standards for Licensed Child Placing Agencies.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Based on review of the record for foster child (FC) FC1 and interview with the foster care program manager, the licensee failed to ensure contacts with the child were current.

Findings:
The most recent contact note documented in the record for FC1 at the time of the inspection was for 3/24/22.

During the licensing inspector's interview with the program manager, it was acknowledged that the contact notes documented in the record for FC1 were not current within 30 days.
