Comments:

An unannounced initial inspection was initiated with a site visit on October 15, 2019 from 11:05 a.m to 1:00 p.m. The inspection was conducted in response to an initial application for licensure. The office space was inspected, four personnel records were reviewed, and background checks for all board officers were reviewed this date. Preliminary findings from the onsite inspection were discussed during an exit interview that was conducted at 12:30 p.m. An acknowledgment form was completed and provided to the agency this date. Desk review of the applicant's policies and procedures was conducted on 10/25/2019, 10/29/2019, and 11/01/2019. A phone exit interview was conducted with the Executive Director, Case Manager, and Recruiter/Trainer on 11/01/2019 from 4:00 p.m. to 4:45 p.m. There were no citations for violation of the Standards for Child Placing Agencies.
