Comments:
An unannounced focused inspection was completed on 2/12/2020 from 10:20 a.m. to 4:40 p.m. and 2/13/2020 from 10:05 a.m. to 1:55 p.m. The licensee reported a total of 16 children in foster care and nine (9) approved foster homes. During this inspection two (2) child and nine (9) foster home records were reviewed. The Director participated in the entrance conference, remained available during inspection, and participated in the exit interview. The Foster Home Caseworker was available during inspection and participated in the exit interview. The exit interview was conducted from 12:15 p.m. to 1:55 p.m. An acknowledgement form was completed and left with the licensee at the end of the inspection.

There were four (4) citations for violations of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:

Based on record review and interview, the agency failed to consider all available information prior to making a decision on Foster Home 2 (FH2).

Findings:

The case record for FH2 contained physical examination reports for both applicants. The forms included statements that each was physically and mentally capable of providing care to children. The exam report for applicant 1 identified six (6) medical conditions and the exam report for applicant 2 identified eight (8) medical conditions. The Home Study did not discuss the agency?s consideration of the medical conditions prior to making a decision to approve the home. The findings were discussed with agency staff during the exit interview. Staff acknowledged the findings.

Plan of Correction: Program manager added addendum to FH 2 home study related to Health Statements providing additional information addressing medical conditions presented on Health statements. Staff had post licensing training to ensure that any families with numerous medical conditions is addressed in home studies on 2/20/20.

Description: Violation:

Based on record review and interview, the agency failed to place a complete Social History in the case record for Foster Child 1 (FC1).

Findings:

The case record for FC1 contained a Social History that did not include information about education and occupation of the child?s aunts, uncles, and grandparents. The fields for this information were blank. The findings were discussed with agency staff during the exit interview. Staff reviewed the Social History and indicated the form was completed on another laptop and provided a copy. Staff acknowledged the completed document was not in the child?s case record at the time of inspection.

Plan of Correction: Program manager added addendum to FH 2 home study related to Health Statements providing additional information addressing medical conditions presented on Health statements. Staff had post licensing training to ensure that any families with numerous medical conditions is addressed in home studies on 2/20/20.

Description: Violation:

Based on record review and interview, the agency failed to address all required elements in the foster home agreements for Foster Child 1 and 2 (FC1 and FC2).

Findings:

The Foster Home Agreements for FC1 and FC2 included language consenting to the foster parents taking the children for routine care and their responsibilities to seek medical care in emergencies. The agreements did not include provisions for receiving consent for emergency medical, mental health, and dental care for the child. The agreement for FC1 was blank for the amount of maintenance to be paid. The findings were discussed with agency staff during the exit interview. Staff reviewed the Foster Home Agreements and acknowledged the findings.

Plan of Correction: The Foster Care Agreement was updated to include provisions for receiving consent for emergency medical, mental health and dental care for the child. Staff training on 2/20/20 reviewed form and discussed need for Foster Care Agreements to have amounts in maintenance to be paid on all contracts.

Description: Violation:

Based on record review and interview, the agency failed to involve the father of Foster Child 1 (FC1) in developing the child?s service plan.

Findings:

The Service Plan for FC1 indicated the child?s mother was not involved in the plan because the mother wasn?t responding to agency communication. The plan did not document reasons the child?s father was not involved in the plan. The case record documented bi-weekly visitation between the child and father via video-conferencing and that the child might return to the father. The case record did not include documentation of agency efforts to contact the father to involve him in the service plan. The findings were discussed with agency staff during the exit interview. Staff acknowledged the acknowledged the findings.

Plan of Correction: Program manager contacted Regional facility and was provided with information on contacting biological parent via mail and sent letter, Initial Treatment, CTP and biological parent info form to local Regional on 2/17/20 with return envelope. Staff had post licensing training to ensure that any incarnated parents are included in treatment on 2/20/20. All treatment plans will had provided to incarnated parents with return postage and means of returning to Imagine Fostering Inc.
