Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
An announced renewal inspection was initiated on 5/19/2020 and concluded on 5/21/2020. The Executive Director was contacted by telephone to initiate the inspection. The agency reported 15 children in placement and 10 approved provider homes. The agency reports one (1) new staff since the last inspection. The inspector emailed the Executive Director the list of cases for review and items required to complete the inspection. The inspector reviewed three (3) child records, one (1) provider home record, and one (1) staff records submitted by the agency to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the agency.

Violations:
Standard #: 22VAC40-131-280-C-4
Description: Violation:

Based on review of the records of Foster Children 1 and 2 (FC1 and FC2), the agency failed to include provisions for receiving consent for routine and emergency medical, mental health, and dental care in the Foster Home Agreements for the children.

Findings:

The records for FC1 and FC2 contained Foster Home Agreements. The Agreements did not include provisions for receiving consent for routine and emergency medical, mental health, and dental care. The findings were discussed during the exit interview. Agency staff S1 and S2 acknowledged the findings and indicated consent was addressed in the agreement between the local department of social services and the licensee and not the Foster Home Agreement.

Plan of Correction: Program Manager- Jon Holmes added provisions in the Foster Parent Agreement for receiving consent for routine and emergency medical, mental health, and dental care in the agreement for children placed. Staff had post licensing training to ensure that new form will be used from 5/26/2020.

Description: Violation:

Based on a review of the record for Foster Child 2 (FC2), the agency failed to involve the birth parents of the child in developing a Quarterly Progress report.

Findings:
The case record for FC2 contained a service plan documenting the child?s goal as Return Home and that contact was permitted between the child and parents. A Quarterly Progress Review report completed 5/6/2020 included a heading titled Include comments and recommendations from: DSS, Foster Parents(s), Biological Parent(s), Service Providers, Child and other members of the treatment team. The report documented the progress review was mailed to (birth mother).? Other documentation of effort to involve the birth mother in the progress review was not located in the case record. The findings were discussed during the exit interview. Staff members S1 and S2 indicate they anticipate receiving feedback from the birth mother and will include this in the progress review. S1 and S2 acknowledged the findings.

Plan of Correction: Program Manager- Jon Holmes updated Initial Treatment, CTP, and Quarterly Progresses, to prompt staff to involve Birth parent in treatment development when child is return home status and parental rights are intact. Staff had post licensing training to ensure that any incarcerated parents are included in the treatment on 5/25/2020. All treatment plans will have provided to incarcerated parents with return postage and means to return to Imagine Fostering Inc. prior to completing on treatment development if phone contact is unavailable.

Description: Violation: 131-350.B.3

Based on record review for Foster Child 1 (FC1), the agency failed to include a description of criteria for achievement and target dates for each goal and objective the written Quarterly Progress Summary for the child.

Findings:

The case record for FC1 contained a Quarterly Progress Summary report dated 4/23/2020. The report discussed the child?s goal and objective related to developmental milestones. The report did not include a description of criteria for achievement for the goal and objective. The findings were discussed during the exit interview. Agency staff S1 and S2 reviewed the report and acknowledged the findings.

Plan of Correction: Program Manager Jon Holmes created prompt in Quarterly to include criteria for achievement and target dates for each goal and objective in the ER version of the Quarterly Progress. Staff were informed that when using form to include or use Word document that includes this information.

Standard #: 22VAC40-131-360-E-3
Description: Violation:

Based on review of the record for Foster Child 3 (FC3), the agency failed to document the name of the individual with whom the child was placed or to whom he was discharged in the child?s comprehensive discharge summary.

Findings:

FC3 was discharged on 3/2/2020. A Discharge Summary, dated 3/2/2020, was included in the child?s record. The Discharge Summary documented the agency did not know the name of the family the child was placed with upon discharge. The report stated the child was discharged to the local department of social services holding custody of the child. The report did not document the name of the individual to whom the child was discharged. The findings were discussed during the exit interview. Agency staff S1 and S2 reviewed the Discharge Summary and acknowledged the name of the specific individual to whom the child was discharged was not documented.

Plan of Correction: Program Manager- Jon Holmes created prompt in discharge to include full name of person child is discharged to or placed with at time of discharge. Staff had post licensing training to ensure that full name of persons are listed when child is discharged or placed with. Addendum to FC3 discharge will include name of persons added by the Caseworker.

Description: Violation:

Based on review of the records for Foster Child 1, 2, and 3 (FC1, FC2, and FC3), the agency failed to document the place of birth on the Face Sheet for each child.

Findings:

The Face Sheets for FC1, FC2, and FC3 did not include place of birth as required. The Face Sheet document for the agency does not include a prompt/field to record the place of birth. The findings were discussed during the exit interview. Agency staff S2 acknowledged the findings.

Plan of Correction: RT/Trainer, Elizabeth McMurray created a place of birth section on Face Sheet in Extended Reach and has added all places of birth for current placements. Staff had post licensing training on update and need to complete Face Sheet with place of birth.
