Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An announced renewal inspection was initiated on 4/29/2021 and concluded on 5/4/2021. The Executive Director responded to the notice of inspection and remained available during inspection. The agency reported 20 children in placement and 12 approved provider homes. No new staff were hired since the last inspection. The inspector reviewed three (3) child records and two (2) provider home records.

There were five (5) citations for violations of the Standards for Licensed Child Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:

Based on record review for Foster Home 2 (FH2), the agency failed to obtain a signed authorization allowing the previous child-placing agencies to release information to the licensee.

Findings:
1) The application for FH2 documented the applicants were previously approved as foster parents through two licensed child placing agencies.
2) The record for FH2 contained a release of information form signed by the applicants.
3) The release of information form did not identify the names of agencies authorized to release information to the licensee.
4) The findings were discussed during the exit interview.
5) Staff 1 and 2 (S1 and S2) reviewed the release of information and acknowledged the findings.

Plan of Correction: RT/Trainer Elizabeth McMurray updated the Release of Information to include all initial backgrounds and included previous child-placing agencies and release was added to Extended Reach.

Standard #: 22VAC40-131-250-L
Description: Violation:

Based on record review for Foster Child 2 (FC2), the agency failed to conduct a pre-placement interview.

Findings:
1. The custodial department of social services of FC2 contacted the agency five (5) days prior to the date the child was placed and indicated the child would need placement upon being discharged from a residential placement.
2. The custodial department of social services for FC2 signed the Imagine Fostering referral form two (2) days prior to the child?s placement.
3. The written assessment for FC2 documented an acceptance decision for the placement of FC2 was made the day prior to the child?s placement.
4. The placement of FC2 did not meet the definition of an emergency placement.
5. The written assessment for FC2 documented no pre-placement interview was conducted as the placement was an emergency.
6. The findings were discussed during the exit interview.
7. S1 and S3 acknowledged the findings.

Plan of Correction: Program manager- Jon Holmes updated Referral Form to prompt staff and LDSS staff to notify Imagine
Fostering of referral's time in custody to determine if standards are followed for emergency or non-emergency placement. Staff had post licensing training on this update on 5/04/21. New form was added to Extended Reach for consistency.

Description: Violation:

Based on a review of the records for Foster Child 1 and 2 (FC1 and FC2), the agency failed to address all required elements in the written reports required by 22VAC40-131-250.O.

Findings:
1. The case record for FC1 and FC2 contained written reports as required by 22VAC40-131-250.O.
2. The written report for FC1 did not address the child?s emotional and psychological problems or information concerning professional treatment needed or received to address emotional and psychological problems.
3. The written report for FC2 did not address the child?s adjustment to school.
4. The findings were discussed during the exit interview.
5. Staff 1 and S3 reviewed the written reports and acknowledged the findings.

Plan of Correction: Program manager- Jon Holmes updated Initial Assessment to give more direction to addressing the emotional and
behavioral needs of child placed, professional treatment needed to address these needs. The child's adjustment to
school was added too as well and new form was placed in Extended Reach to start being used. Staff had post licensing training on this update 05/04/2021

Standard #: 22VAC40-131-260-B
Description: Violation:

Based on record reviews for Foster Child 1 and 2 (FC1 and FC2), the agency failed to address all required elements in the Social Histories.

Findings:
1. The Social History for FC1 and FC2 did not include names and addresses of providers of medical treatment or documentation of the licensee?s attempt to obtain the information.
2. The Social History for FC1 documented the education and occupation of the child?s parents, aunts, uncles and grandparents as ?unknown to DSS.? The agency?s effort to obtain the information was not documented.
3. The Social History for FC1 documented the medical and psychiatric history of the child?s mother and grandmother as ?N/A.? For the father, aunts, and uncles the response was ?unknown to DSS.? The case record documented the agency?s communication with the birth mother. The case record did not document efforts to obtain the information.
4. The Social History for FC1 and FC2 did not document the needs of the child.
5. The findings were discussed during the exit interview.
6. S1 and S3 reviewed the documents and acknowledged the findings.

Plan of Correction: Program manager - Jon Holmes updated the Social History form to include names and addresses of providers of medical treatment and need for documentation of attempts to gather information. Social History was updated to include attempts to gather education and occupation of the child's parents, aunts, uncles and grandparents. Social History was updated to include the Needs of the child. The updated Social History was placed in Extended Reach. Staff had post licensing training on this update on 5/04/21. New form was added to Extended Reach for consistency.

Standard #: 22VAC40-131-340-B-1
Description: Violation:

Based on record reviews for Foster Child 1 and 2 (FC1 and FC2), the agency failed to include measurable objectives and strategies describing services to be provided to the children, placement families, and birth families during the first 45 days of placement.

Findings:
1. The agency?s Intake Assessment document includes a question to address the requirement, for children in treatment foster care, to develop and implement individualized, measurable objectives and strategies describing services to be provided to the child, his placement family, and his birth family during the first 45 days of placement.
2. The Intake Assessment documents for FC1 and FC2 included information that was not measurable or descriptive of services to be provided to the child, the placement family, or the birth family during the first 45 days of placement.
3. The findings were discussed during the exit interview.
4. S1 and S3 reviewed documentation and acknowledged the findings.

Plan of Correction: Program manager- Jon Holmes added prompt to Initial Assessment to include measurable and description of services to be provided to the child, placement family and birth family during the first 45 days and the updated form was placed in Extended Reach with staff having post licensing visit training on changes.
