Comments:

A monitoring inspection was initiated on 10/22/2021and concluded on 10/28/2021. The Executive Director was contacted by telephone to initiate the inspection and reported that the current census was 19. The inspector emailed the Executive Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed two (2) foster child, three (3) staff, and two (2) provider home records. An exit interview was conducted with the Director, Case Manager, and Recruiter/Trainer on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description:	Violation: Based on interview, the licensee failed to maintain required documentation in the records of Personnel 2 and 3 (P2 and P3). Findings: 1) The Licensing Inspector requested documentation of current annual performance evaluations for P2 and P3. 2) Documentation of current annual evaluations for P2 and P3 were not received by the Licensing Inspector. 3) The findings were discussed during the exit interview. Staff 1 (S1) stated the annual evaluations for P2 and P3 are pending completion and are not available.

Description:	Violation: Based on record review and interview, the licensee failed to consider all information received about Applicant 1 (A1) in Foster Home 2 (FH2) prior to making a decision on the applicant. Findings: 1) A health statement for A1 included the question ?Does this individual have a history of mental illness, alcohol, or substance abuse?? The question was answered yes. 2) The home study for FH2 stated ?No mental illness or substance abuse were reported. There were no other concerns reported on the health statement.? 3) The findings were discussed during the exit interview. Staff reviewed the health statement for A1 and acknowledged the findings.

Description:	Violation: Based on record review and interview, the licensee failed to address required elements in the Individualized Service Plan for Foster Child 2 (FC2). Findings: 1) The Individualized Service Plan for FC2, dated 8/20/2021, did not include a target date for discharge from the program. 2) The findings were discussed during the exit interview. Staff reviewed the documentation and acknowledged the findings.

Description:	Violation: Based on record review and interview, the licensee failed to address required elements in a Quarterly Progress Summary for Foster Child 2 (FC2). Findings: 1) A Quarterly Progress Summary, dated 10/5/2021, for FC2 did not document the child?s assessment of his progress. The document included a field to document this information. The field stated ?[FC2] is happy in the [foster home].? 2) The findings were discussed during the exit interview and staff acknowledged the findings.
