Type of inspection: Renewal
Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection:
4/11/2023 from 11:00 a.m. to 4:48 p.m.

A preliminary findings review was conducted and an Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. An exit interview was conducted by Teams Meet on 4/17/2023.

Number of children in care: 20
Number of approved provider homes: 16
The licensing inspector completed a tour of the physical plant that included the office setting and conditions.

Number of Provider Home records reviewed: 2

Background checks for board officers were reviewed.

The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.

Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.

Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.

The department's inspection findings are subject to public disclosure.

Description: Violation:

Based on record review, the agency failed to ensure all materials and information received by the licensee indicated the date received.

Findings:
1) The Division of Motor Vehicle (DMV) reports for applicants in Foster Home 1 did not indicate the date the reports were received.
2) The findings were discussed during the preliminary findings review.
3) Staff reviewed the DMV reports and acknowledged the findings.
4) The findings were reviewed during the exit interview and Staff 1 (S1) acknowledged the findings.

Description: Violation:
Based on record review, the agency failed to obtain a written medical examination report addressing required elements for Foster Child 1 (FC1).

Findings:
1) FC1 was placed with the agency on 8/29/2022.
2) The record for FC1 contained a medical examination report dated 8/23/2022 that did not address nutritional status, chronic conditions, or disabilities.
3) The record for FC1 contained a medical examination report dated 10/6/2022 that did not address nutritional status, freedom from tuberculosis and communicable disease, chronic conditions, or disabilities.
4) A quarterly progress review for FC1 dated 11/25/2022 documented FC1 was seen for a physical exam on 10/27/2022.
5) The record for FC1 did not contain a medical examination report regarding the physical exam on 10/27/2022.
6) The findings were reviewed during the preliminary findings review.
7) Staff reviewed the record and physical exam reports for FC1 and acknowledged the findings.
8) The findings were reviewed during the exit interview and Staff 1 (S1)
