Comments:
A renewal inspection was initiated on July 13, 2021 and concluded on July 22, 2021. The Executive Director was contacted by telephone to initiate the inspection. The Executive Director reported that the current census was (10). The inspector emailed the Executive Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed 2 youth records and 2 staff records submitted by the facility to ensure documentation was complete. An exit meeting was conducted with the Executive Director on July 22, 2021 where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and a violation were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-131-40-B
Description: Violation:
Based on review of the record for a youth placed in an independent living arrangement, Youth 2, the agency's policies and procedures, and interview with agency representative AR2, the agency failed to comply with its own policies and procedures.

Findings:
1. Review of the agency's policies and procedures revealed the following procedure: "No later than 30 days after the date of discharge, ILS will develop and place a comprehensive discharge summary in the youth's file".
2. AR2 acknowledged that while the discharge summary packet was started when Y2 discharged in April 2021, it was not completed.

Plan of Correction: Executive Director will provide additional training during supervision regarding the importance of following through with discharge summary regardless of length of stay in the program to Program Staff.
