Comments:

A monitoring inspection was initiated on 10/25/21 and concluded on 11/03/2021. The Program Manager and the Executive Director were contacted by telephone to initiate the inspection. The Program Manager reported that the current census was two (2) children and three (3) foster homes. The inspector emailed the Program Manager a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed two (2) foster home records, one (1) foster child's record, and two (2) staff records. An exit interview was conducted with the Program Manager and the Executive Director on 11/03/21, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the agency.

Description:	Violation: Based on a review of the "Intake and Pre-Placement Form" submitted by the agency for a foster child (FC-1) and an interview with an agency representative (AR-1), the agency failed to include required information in their intake assessment. Evidence: 1) The "Intake and Pre-Placement Form" for FC-1 dated 9/06/21 did not include the date of planned achievement for the child's permanency planning goal. 2) In an exit interview on 11/03/21, AR-1 acknowledged that the section of the form for the date of planned achievement for the child's permanency planning goal was left blank.

Description:	Violation: Based on a review of the placement documents submitted by the agency for a foster child (FC-1) and an interview with an agency representative (AR-1), the agency failed to obtain a written foster home agreement with the foster family (FH-1). Evidence: 1) FC-1 was placed with FH-1 on 9/10/21. There was no foster home agreement in the family's file. 2) During an exit interview on 11/03/21, AR-1 acknowledged that there was no foster home agreement in the family's file for FC-1.

Description:	22VAC40-191-40.C.3 Violation: Based on a review of the background checks submitted by the agency for a foster family (FH-2) and an interview with an agency representative (AR-1), the agency failed to obtain sworn statements for both foster parents of FH-2 prior to approval. Evidence: 1) FH-2 was approved as a foster family on 7/01/21. FH-2 is a two parent foster home. 2) No sworn statements were submitted to this licensing specialist for review. 3) In an exit interview on 11/03/21, AR-1 acknowledged that they were not able to locate the sworn statements for either parent of FH-2.
