Comments:

Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 6/16/23 from 10:45 AM ? 11:11 AM ? Inspection of the office setting and conditions 6/28/23 from 10:05 AM ? 4:48 PM ? Remote inspection of records and preliminary findings meeting The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Number of youth in care: 13 The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of youth records reviewed: 2 Number of staff records reviewed: 4 Number of interviews conducted with staff: 4 Additional Comments/Discussion: An entrance conference was conducted by phone on 6/28/23 with the Program Manager. Also, on the same date, youth records and staff records were reviewed remotely. The Program Manager was available and accessible during the inspection. A preliminary findings meeting was conducted on the same date by phone. The Program Manager was interviewed to address questions about documents reviewed in the youth records. The Human Resources Manager was interviewed to address questions about documents reviewed in the staff records. An exit meeting was conducted on 7/13/23 to review the inspection findings. The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary.
