Type of inspection: Renewal Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 8/31/23 from 2:04 PM ? 6:25 PM 9/1/23 from 10:35 AM ? 5:01 PM The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Number of youth in care: 12 The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of youth records reviewed: 2 Number of staff records reviewed: 5 Number of interviews conducted with staff: 2 Additional Comments/Discussion: An entrance conference was conducted at the agency on 8/31/23 with the Site Supervisor. Also, while at this agency, youth records and staff records were reviewed. The Site Supervisor was available and accessible during the inspection. A preliminary findings meeting was conducted on 9/1/23 with the Site Supervisor and the Case Worker. An exit meeting was conducted on 9/6/23 to discuss the inspection findings. The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
