Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 5/2/2024 from 12:25 PM ? 5:00 PM 5/9/2024 from 3:30 PM ? 4:30 PM The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Number of youth in care: 7 The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of youth records reviewed: 2 Number of staff records reviewed: 5 Number of interviews conducted with staff: 2 Additional Comments/Discussion: An entrance conference was conducted at the agency on 5/2/2024 with the Site Supervisor and Caseworker. Agency staff was not present initially upon inspectors? arrival but arrived shortly thereafter. Licensing inspector introduced self as primary inspector for this Tidewater location. Inspector was assisted by Independent Living Program Director via electronic communication and site supervisor who was in person during the visit. Inspector was given a tour of the office location and settings and reviewed posted license on wall with most recent inspection summary. Inspector reviewed youth and staff records via Extended Reach EHR system. Agency has not employed any new staff since last inspection visit and no policy updates. A preliminary finding meeting and exit meeting was rescheduled from 5/9/2024 and conducted on 5/14/2024 at the office location with Site Supervisor and IL Program Director who participated by phone. An exit meeting was conducted on 5/14/24 to discuss the inspection findings. The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
