Type of inspection: Renewal An on site inspection was conducted on 09/07/2023 from 11:30 am until 3:30 pm The Acknowledgement of Inspection form was signed. Number of children in care: 3 Number of approved provider homes: 2 The licensing inspector completed a tour of the physical plant that included the office setting and conditions. yes Number of children?s records reviewed: 3 Number of staff records reviewed: 7 staff records Number of interviews conducted with staff: 1 entrance and exit interview with the Director of Foster Care. Observations by licensing inspector: The office setting and conditions are in compliance with the regulations. Additional Comments/Discussion: Lutheran Family Services of Virginia received a conditional license on 4/10/23 to provide transitional foster care services to undocumented children and youth at this location. An exit interview was conducted on the date of the inspection. The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
