Type of inspection: Initial Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 10/10/2023 from 11:00a.m. to 12:30 p.m. and the exit meeting was completed on the same day. The Licensing Inspector met with the President and the Admissions Specialist to initiate the inspection. The Licensing Inspector reviewed the initial application and addressed in any issues with the potential licensee. Number of approved provider homes: (0) Number of children in care: (0) Number of children?s records reviewed: (0) Number of provider home records reviewed: (0). Number of staff records reviewed: (5). Observations by licensing inspector: The Licensing Specialist observed the office space to ensure that it met all requirements related to standards regarding Office Settings and Conditions (22VAC40-131-50 B). There were no major changes made to the policies or procedures regarding operations since making the application for the new location. The evidence gathered during the inspection determined (0) violations with applicable standard(s) or law.
