Comments:

An unannounced monitoring inspection was conducted on 12/20/2021 from 11:15 a.m. to 3:50 p.m. The Executive Director was contacted and a COVID screening was conducted prior to the inspection. The Executive Director and Program Director participated in the entrance conference and remained available during inspection. The facility reported the current census to be six (6) residents. The inspector reviewed two (2) resident records and Medication Administration Records, three (3) staff records, a sampling of menus, daily communication logs, staff work schedules, and fire drills to ensure documentation was complete. One resident and one staff member were interviewed. A tour of the facility was conducted the environment was inspected. An exit interview was conducted with the Program Director. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
