Comments:

An onsite monitoring inspection was conducted on 3/9/2022 from 10:55 a.m. to 4:30 p.m. and 3/10/2022 from 10:25 a.m. to 2:15 p.m. An entrance conference was completed with the Executive Director/Program Director, Human Resources Director, and Program Licensing and Staff Assistant. Staff reported a current census is nine (9) residents. The inspector reviewed documentation from five (5) resident records, and three (3) personnel records. The inspector also reviewed the fire inspection, sanitation inspection, emergency evacuation drill documentation, daily log book entries, six (6) medication administration records, readily accessible medical emergency information, case notes, menus, and staff work schedules. Two (2) residents and four (4) staff were interviewed. An exit interview was conducted with the Executive Director/Program Director, Program Licensing and Staff Assistant, and Human Resources Director on 3/10/2022, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined no non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.
