Comments:

An unannounced monitoring inspection was conducted on September 26,2019 from 11:00 a.m. to 3:15 p.m. Three personnel records, to include background investigation information, was reviewed. Two residents were in care and both residents' records were reviewed. No residents have discharged. Other documentation reviewed at this inspection included but was not limited to menus, house parent log notes, emergency drill documentation, counseling reports, medical care documentation, and medication administration records. Interior and exterior physical plant inspections were conducted. The Program Director was present and available to respond to question during the inspection. A staff and resident were interviewed. Exit meeting was held with the Program Director to discuss preliminary inspection findings before leaving the facility on September 26, 2019. An acknowledgement form was signed at the exit meeting
