Type of inspection: Monitoring Visit Date(s) of inspection and time the licensing inspector was on-site at the facility. 5/30/2024 10:00AM-11:30AM The Acknowledgement of Inspection form was signed and left at the facility on date of the inspection. Number of residents in care at the beginning of the inspection: 2 The licensing inspector completed a tour of the physical plant location. Number of resident?s records reviewed: 2 Number of staff records reviewed: 1 Number of interviews conducted with staff: 1 Additional Comments/Discussion: A monitoring inspection was initiated on 5/29/24 and the administrator was contacted by telephone on this date to initiate the inspection. The administrator reported that the current census was 2. The inspector reviewed (2) resident records, (1) staff records. The following information was reviewed for youth records, monthly progress notes, quarterlies, most recent medical examination, and service plans. There were no serious incidents involving the youth to report. Licensing inspector reviewed the following documents from the employee record, background/sworn statements, trainings, performance evaluation, and tuberculosis screening assessments. No new employees have been hired since last inspection. Copies of fire and health inspection were reviewed and remain current as 5/30/2024. An exit interview was conducted with Administrator/house parent on 5/30/24, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.
