Comments:

Type of inspection: Renewal The Licensing Inspector was on-site at the facility for the unannounced renewal inspection on April 30, 2024, from 12:30 PM to 3:20 PM. The Acknowledgement of Inspection form was signed and left at the facility the day of the inspection. Number of residents in care at the beginning of the inspection: 8 The licensing inspector completed a tour of the physical plant to include the residents? sleeping arrangements. Number of resident?s records reviewed: 2 (includes 1 discharge) Number of staff records reviewed: 5 Number of interviews conducted with residents: 0 Number of interviews conducted with staff: 1 Observations by licensing inspector: The Executive Director was available and accessible throughout the duration of the inspection. Overall, the program services appear to be well organized and meets standards. Additional Comments/Discussion: An entrance conference was held on 4/30/24. Reviewed residents staff records to ensure required documentation including background checks were observed in staff records. Reviewed and discussed standards that were the focus of the renewal inspection onsite visit. The annual Fire and Health inspections on file observed to be current as well as Insurances. Licensing Inspector also reviewed a copy of the Annual Quality Improvement Plan. An exit meeting was conducted on the date of the onsite inspection of 4/30/2024. The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary.
