Comments:

A complaint inspection was initiated on 8/19/21 and concluded on 9/3/21. A complaint was received by the department regarding an allegation in the area of Structured Program of Care. The Executive Director was contacted by telephone to conduct this investigation. The Licensing Inspector emailed the Executive Director a list of documentation required to complete the investigation. The Executive Director reported the current census was three (3) residents. Violations were not cited in relation to this complaint. Therefore, this complaint is determined not valid. The following is a listing of the activities for this inspection: The record for one youth was reviewed. The Executive Director was interviewed on 8/19/21. The licensee should retain a copy of this document to be posted at the facility. The results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services? website within 5 business days.
