Comments:

An announced initial inspection was completed by two Licensing Specialists on September 3, 2019 from 9:21 a.m. to 4:21 p.m. The Chief Administrative Officer was interviewed by phone on September 4, 2019. Desk reviews of additional documentation were conducted on September 24, 2019 and October 9, 2019 with the findings reviewed with the Program Director on October 9, 2019. The applicant is seeking licensure from the department; therefore, this inspection was conducted as a result of the submission of their initial application. The following is a listing of the activities for this initial inspection: an inspection of the interior and exterior of the facility was completed to include measuring bedrooms, fourteen personnel records, which included background investigations, were reviewed, the Program Director and Program Manager were interviewed during the inspection, and the facility's policies and procedures manual and forms were reviewed. An acknowledgement of inspection form was signed and left at the facility.
